Pain Review & Referral
Helping patients manage chronic pain. Our specially trained Pain Review Team works with your primary provider to help you explore treatment options.
Women, Infants & Children (WIC)
Supplemental foods, health referrals and nutrition education for low-income pregnant, breast-feeding and postpartum women and children under age 5.
MI Care Team
The MI Care Team service model helps chronically ill Medicaid and Healthy Michigan Plan beneficiaries better manage their conditions. Qualified patients are those with depression/and or anxiety plus one or more of these conditions: heart disease, chronic obstructive pulmonary disease, hypertension, diabetes or asthma.
